			<content:encoded><![CDATA[<p>A certified nursing assistant (CNA) at a Limestone nursing home was charged Friday by the Washington County Sheriff&#8217;s Department with five counts of willful abuse, neglect or exploitation of an adult, according to a spokesman for the Washington County Sheriff&#8217;s Department.</p>
<p>Amanda Tibble, 34, of 112 George Laws Road, Telford, was charged &#8220;after an investigation was conducted into physical and emotional abuse at John M. Reed Nursing Home&#8221; in Limestone, said Washington County Sheriff Ed Graybeal in a news release.</p>
<p>The Johnson City Press reported, &#8220;The abuse apparently primarily involved Tibble&#8217;s using profanity toward residents &#8230; but there was at least one occasion when Tibble became physical with one resident at the nursing home.</p>
<p>&#8220;The defendant became aggrsssive with a 75-year-old client by twisting his arm behind his back and was using profanity towards the client.&#8221;</p>
<p>The Johnson City Press said Investigator Mike Adams &#8220;wrote that Tibble admitted to being verbally abusive to four clients by using profanity directed to them.&#8221;</p>

			<content:encoded><![CDATA[<p>A Fairfax County jury has awarded nearly $3 million to the family of a man who died after his esophagus tore while he was swallowing a piece of steak, finding an Alexandria radiologist liable for misdiagnosing the man&#8217;s condition as a hiatal hernia.</p>
<p>Large civil jury verdicts are rare in Fairfax, and Virginia&#8217;s cap on medical malpractice judgments required the jury&#8217;s award of $2,933,500 to be cut by more than half, to about $1.25 million.</p>
<p>Just before trial, the Fairfax anesthesiology group involved in the man&#8217;s death agreed to pay a settlement of $600,000.</p>
<p>Hector Alvarez, 52, of Springfield first went to the Inova HealthPlex at Franconia-Springfield on July 6, 2006, complaining of chest pains and trouble swallowing after he had barbecued a steak and taken a bite. The piece of meat had gotten stuck in his throat, attorney John J. Sellinger said, and &#8220;he later said it felt like it went down the wrong way.&#8221;</p>
<p>According to court records, Alvarez was given a number of tests at Inova HealthPlex, including X-rays, an EKG and a CT scan of his chest. The CT studies were sent electronically to William J. Dunwoody III, a radiologist with the Association of Alexandria Radiologists, who was working that evening at Inova Alexandria Hospital, Sellinger said.</p>
<p>Dunwoody&#8217;s computer notes indicated that his impression was that Alvarez was suffering from &#8220;a large hiatal hernia,&#8221; according to court papers. Sellinger said doctors continued to treat Alvarez with various oral medicines, but his pain did not decrease. Dunwoody and his attorney, Stephen L. Altman, did not return calls for comment.</p>
<p>About midnight, Alvarez was taken by ambulance to Inova Fairfax Hospital. Doctors continued to search for the cause of his severe chest pain and continued to give him oral treatment. About 4 p.m. on July 7, Sellinger said, a surgeon looked at Alvarez&#8217;s CT scan and spotted the perforated esophagus.</p>
<p>The surgeon ordered that Alvarez be immediately prepared for an operation to repair his esophagus. The surgeon also ordered that Alvarez be given an epidural catheter, so that he could receive pain medication after the operation, Sellinger said.</p>
<p>But after the catheter was inserted and anesthetic applied, Alvarez went into cardiac arrest, records show. Court documents show that a doctor at Fairfax Anesthesiology Associates performed the procedure. The attorney for the anesthesiology firm, Richard L. Nagle, did not return calls.</p>
<p>Sellinger said Alvarez had been severely weakened by having a perforated esophagus for nearly 24 hours, and &#8220;if he hadn&#8217;t been so sick when they put the epidural in, he wouldn&#8217;t have arrested.&#8221;</p>
<p>There was difficulty resuscitating Alvarez, Sellinger said, and he suffered irreparable brain damage. The surgery was not performed.</p>
<p>Alvarez died July 17, 2006.</p>
<p>He had retired from a career in the Air Force and worked as an information technology specialist for the Defense Information Systems Agency, where Sellinger said Alvarez&#8217;s salary was $100,000. He was married with two adult children and was &#8220;an incredibly nice guy,&#8221; Sellinger said.</p>
<p>Alvarez&#8217;s widow, Deborah A. Alvarez, filed a wrongful death suit in March 2007, naming Dunwoody, the Association of Alexandria Radiologists and Fairfax Anesthesiology as defendants. Neither the individual anesthesiologist nor Inova was named.</p>
<p>After a lengthy pretrial process, Fairfax Anesthesiology agreed in January to pay $600,000, court records show. But the Alexandria radiology firm, which Sellinger said was insured by ProAssurance, did not make a settlement offer and elected to go to trial.</p>
<p>The trial began early this month before Fairfax Circuit Court Judge Jane Marum Roush. It was postponed for a week by snow.</p>
<p>The trial resumed Feb. 16, when the jury issued its verdict against Dunwoody and the Association of Alexandria Radiologists. The jury was not told of the state cap on medical malpractice awards, which is $1.85 million for actions that occurred in 2006.</p>

			<content:encoded><![CDATA[<p>Members of a Spanaway church are mourning a pastor who died Saturday after his motorcycle struck a car in Roy.</p>
<p>Thom Carlson, 43, of Puyallup was on his bike at the 32100 block of Eighth Avenue South when he hit a vehicle pulling into the parking lot of the Lacamas Community Club. The motorcycle had been traveling at a high rate of speed, according to Pierce County sheriff’s spokesman Ed Troyer. Carlson was taken to a hospital, where he died of his injuries.</p>
<p>Carlson served as pastor for six years at the Real Life Family Center in Spanaway. His congregation gathered to mourn the loss of the avid outdoorsman, who spent seven years in the Army and worked as a diesel mechanic and ranch hand in Montana before receiving his calling.</p>
<p>H.R. Huntsman, the lead pastor of the church, described Carlson as warm and outgoing. “We spent a lot of time riding motorcycles together,” Huntsman said. “Thom and I were very good friends, closer to each other than we were to our own brothers.”</p>
<p>Carlson leaves behind Sheri, his wife of 21 years. His family and congregation are planning a memorial service.</p>

			<content:encoded><![CDATA[<p>A Federal Way family of four has been taken to a hospital after complaining of headaches in a possible case of carbon monoxide poisoning.</p>
<p>South King Fire and Rescue spokeswoman Kendra Kay says medics responded Friday night to find that power in the apartment building was out and the family had lit a charcoal grill on their deck.</p>
<p>Kay says the wind likely blew some carbon monoxide from the grill into the apartment through an open sliding door.</p>
<p>The two adults and two young children were all conscious. KOMO-TV says they were taken to Virginia Mason Medical Center in Seattle to be treated in a hyperbaric chamber.</p>

			<content:encoded><![CDATA[<p>Seattle, WA—A teenager was arrested after he drove a SUV through Shorecrest High School, 75-feet down the hallway and into a security office. The 17-year-old teenager crashed through the high school around 10:00 p.m., in King County, as reported by The Seattle Times.</p>
<p>The King County Sheriff’s Office reported that the teen drove a 2002 Subaru Outback through the doors of Shorecrest High School, down the hall about 75-feet and crashed into a security office. Police located the teen about an hour and half later on the school property after running the tags on the car, and discovered that the car belonged to the boy’s parents. The SUV was reportedly found with broken walls on top of the vehicle. The boy was arrested by police officers an then taken to Harborview Medical Center “for a medical evaluation.” Drugs and/or alcohol do not appear to be factors. Thankfully, there was no structural damage to the building, nor any reported injuries. The car had to be pushed back down the hallway of the school to get the car out of the building; the teen’s parents later retrieved it. A full investigation is underway. It was not reported what the boy was charged with.</p>

			<content:encoded><![CDATA[<p>A 2-year-old boy died inside a home in the city’s South Ward tonight, in what appeared to have been an &#8220;unfortunate accident&#8221; involving fallen furniture, police said.</p>
<p>Officers responding to reports of an injured child in the 300 block of Clinton Place entered a home to find a 2-year-old child unconscious and unresponsive shortly after 8 p.m., said city police Detective Hubert Henderson. The child was taken to University Hospital, where he was pronounced dead a short time later.</p>
<p>The child was apparently watching television with his older brother when he opened one of the dresser drawers, causing both the dresser and the television to fall. The dresser and the television struck the younger child, causing serious injuries, Henderson said.</p>
<p>The boy’s mother was not home at the time of the incident, according to Henderson, and his grandmother was watching television in the front room of the house.</p>
<p>While preliminary details suggest the boy’s death was an accident, police remained at the scene tonight making a final determination, Henderson said.</p>
<p>Wrongful Death in Washington, RCW 4.20.010, provides for a cause of action in the event of a wrongful death of a family member and loved one. When the death of a person is caused by the wrongful act, neglect or the fault of another, his or her personal representative may maintain an action for damages against a person or entity which is responsible for causing the death of another. <p>Several staff members at the Schofield School thought they were being given the H1N1 vaccine on Friday. Instead, they were given a shot of insulin.</p>
<p>Many people were shocked by the news.</p>
<p>“When I heard this, I was kind of taken aback,” said Jackie Norton, a parent.</p>
<p>In a letter to the school&#8217;s community, Bella Wong, the Superintendent of Wellesley Schools, said: &#8220;It is clear a very serious error did occur, and we will be fully investigating this event…Once the error was discovered, an immediate attempt was made to reach all staff that had received any shots at school on Friday.”</p>
<p>Diabetics take insulin to regulate their blood sugar.</p>
<p>“They’re trying to handle all these different shots and I can see…there’s enough of them going around, something’s bound to happen,” said Mark Webb, who works in Wellesley.</p>
<p>The insulin came from the supply on-hand at the school for diabetic students. No students were impacted. It is still unclear how many staff members were affected.</p>
<p>Those who received the insulin shots were treated at Newton-Wellesley Hospital, and all are now fine.</p>
<p>The person who administered the shots is now on paid leave.</p>

			<content:encoded><![CDATA[<p>A pedestrian was struck and killed by a semi-truck on Interstate 5 in Thurston County early Tuesday morning.</p>
<p>The State Patrol reported that the victim had been standing in a northbound lane near mile post 97 when he was hit. The 24-year-old man was from La Center in Clark County.</p>
<p>The 64-year-old truck driver from Oregon was not injured in the accident.</p>
<p>The Patrol said neither drugs nor alcohol were believed to be a factor in the accident that happened shortly before 1:00 a.m.</p>

<p>A car struck and killed a pedestrian on Blaine Road just before 3:00a.m. on Saturday morning.</p>
<p>The scene was south of the Lynden-Birch Bay Road.</p>
<p>A witness describes the car as a dark colored sedan and detectives believe it may have visible front-end damage.</p>
<p>Investigators are asking anyone with information on the car or driver involved in the fatality in Whatcom County last weekend to please contact them.</p>

<p>Eric Davis, pierced by a 3-inch nail from a nail gun, was released from a Boise hospital Christmas Day.</p>
<p>The gun bounced off a ladder Tuesday at a construction job at the Tamarack Resort in Valley County, according to KIVI-TV (Channel 6). “My hand just slipped and hit the trigger,” Davis told the station.</p>
<p>“He could feel it move every time his heart would beat,” said his wife, Jennifer Cafferty-Davis.</p>
<p>No one had pulled out the nail, and that kept him from bleeding to death, Cafferty-Davis said. Davis was taken by ambulance to a McCall hospital, then flown to Saint Alphonsus Regional Medical Center in Boise. There, surgeons opened his chest cavity, his wife said.</p>
